Designs for Uploader error handling

Hi Mike and Harris,

I've made a wiki page to host the wireframes and description of how Uploader
should handle both client- and server-side errors (for FLUID-3878 and
related JIRAs):

The designs for error handling fit on top of the existing Uploader designs,
so non-error interactions with uploads shouldn't be affected.
